On Your First Visit Please be sure to bring your Doctor's prescription (Rx) for physical therapy. Although some insurance plans do not require a prescription (also called a "referral"), NY State law requires Physical Therapists to have a prescription from the patient's physician to begin physical therapy. The prescription must include your doctor's diagnosis (Dx). Prescriptions can be faxed directly from the physicians office to our office, our fax number is (646) 497-1459. Bring your insurance card(s), as we'll make a photo-copy for your chart. Dress comfortably. It is not necessary to bring sweatpants or work-out clothes to your first session, unless your Physical Therapist has asked you to do so. If you bring a change of clothes, we provide dressing rooms with lockers to store your items during your session. |
Insurance Plans When you call for the first time,
be ready to provide your name, date-of-birth, insurance plan name and
ID#, and the "provider services" phone # for your plan. This
allows us to verify your benefits with your provider. We'll
call you back the same day, and give you the
